Schedule an Appointment

If your clinician has given you an order or referral for a procedure, or if you would like to schedule a Mammogram or Bone Densitometry Scan (DEXA):

  • Call us at 360-493-4646 to schedule an appointment now.

If you have a referral slip from your physician, please bring it in at the time of your exam along with your insurance information.

If you need to cancel or reschedule your exam, please be sure to call our office at 360-493-4646 so we may schedule you for another time.

Registration

To simplify and expedite your registration process, we have added patient registration forms to our website. You may download these PDF forms to your home computer, fill in your information, print the form, and bring it with you when you come for your appointment. You may also print the blank form and fill it out by hand.

These online registration options have been added for your convenience. Of course, you are still welcome to simply pick up the forms and fill them in at the time of your appointment.
