To simplify and expedite your registration process, we have added patient registration forms to our website. You may download these PDF forms to your home computer, fill in your information, print the form, and bring it to us when you come for your appointment. You may also print the blank form and fill it out by hand.   Of course, you are still welcome to simply pick up the forms and fill them in at the time of your appointment.

Patient Registration

All patients must complete this form prior to each appointment. (Please note: patients who have completed this form for past appointments must still fill out a new one so that we have the most current information.)

Please remember, this form must be signed in the presence of a technologist at the time of your exam.

Medication Reconciliation Form

For patients requiring sedation prior to their exam, this form must be completed with the medication that the patient is presently taking.
